Nico e94f1c437c Add Streamable HTTP transport for MCP (replaces SSE as primary)

- New /mcp endpoint with StreamableHTTPServerTransport (stateless per-request)
- SSE kept as /mcp/sse fallback
- Survives pod restarts — no persistent connection needed
